I seem to have read about someone who had designed a way for a hydraulic cylinder to produce a continuous rotating output.

Can anyone help with more info on this?

What exactly do you mean by continuous? One-way indexing, or actual continuous motion?

In 1966 when I started working for Miller Fluid Power in Bensenville, Il, on their Plant Tour, they showed a Hydraulic Cylinder Drive for a large Cylinder Tube Hone that was driven by three Double Acting Cylinders attached to a Crankshaft. It ran around 30-50 RPM and Honed Tubes up to 20" Dia.

It also served as an endurance tester for new materials life span since cycles were numerous over a short period and under load.
